Without 24 hour’s notice, I will charge you 50% of your full session fee. For missed appointments (i.e. you do not show up for your scheduled appointment without cancelling it), you will be charged the full session fee of $175.
Starting March 1, 2022, I will be an out-of-network provider for all Blue Cross Blue Shield plans thus I am not taking on new BCBS clients at this time.
At this time, I am an out-of-network provider for other insurance companies. However, I will provide a receipt for you to submit an out of network claim for reimbursement. Filing for insurance reimbursement is the responsibility of the client. Insurance companies typically reimburse 40-70% of the original fee depending on your specific insurance plan. Some insurance companies allow you to use your Flexible Spending Account if you have one. I recommend that you contact your insurance company prior to our first appointment so that you are aware of your out-of-pocket costs.
How to Submit an Out-of-Network Claim
1. Have your insurance member number and group number ready before calling.
2. Call the customer service number for your insurance for Mental Health and Behavioral Health Claims This should NOT be under Medical Claims, otherwise it could cause a delay in your claim getting processed correctly.
2. Ask the customer service representative:
a. Where to send the information for processing.
b. What information they need for processing.
3. If your insurance company needs additional clinical information about you other than what is provided on your receipt, then I will need a signed release of information from you in order to release any other clinical information besides a diagnosis code.
Financial & Office Policies:
Other Fees: Other services will be charged a pro-rated amount including report writing, telephone conversations lasting longer than 15 minutes, consulting with other professionals with your permission, preparation of records or treatment summaries, and the time spent performing any other service you may request of me. If you become involved in legal proceedings that require my participation, you will be expected to pay for all of my professional time, including preparation and transportation costs, even if I am called to testify by another party. Because of the complexity of legal involvement, I charge $200 per hour for preparation and attendance at any legal proceeding.
Cancellations: Please contact me and leave a message on my voicemail at least 24 hours ahead of time if you need to cancel an appointment. Your appointment time is reserved for you so I cannot use it for other purposes unless I have sufficient notice. Without 24 hour’s notice, I will charge you 50% of your full session fee. For missed appointments (i.e. you do not show up for your appointment without cancelling it), you will be charged the full session fee of $150.
Emergencies: Although I am committed to checking messages and returning calls, I do not carry a pager and I do not provide crisis, 24-hour or weekend coverage. If you feel you are having a mental health emergency or if you feel unsafe and are unable to contact or speak with me, please dial 911 or go to the nearest hospital emergency room, where psychiatric emergencies services should be available or call The North Texas Behavioral Health Authority Crisis-line at 866-260-8000.
