How to Request Health Records

Here are detailed instructions on how to request your health care records:

1.     Email me Dr. Fatter at info@daphnefatterphd.com your request for your records including your full name and date of birth.

2.     Dr. Fatter will email you a consent form for you to sign to release your records.

3. Your records will be distributed to you or to your requested provider to coordinate care via HIPPA compliant email in PDF format.
